=== Maternal paracetamol use === On 24 September 2025, the World Health Organization stated there is no conclusive scientific evidence linking paracetamol (acetaminophen) use during pregnancy to autism. Across multiple international health authorities, including the Society of Obstetricians and Gynaecologists of Canada, the Medicines and Healthcare products Regulatory Agency in the UK, and the European Medicines Agency, paracetemol is recommended as the first-line treatment for pain and fever during pregnancy when used at the lowest effective dose for the shortest duration. == History == The War Office developed from the Council of War, an ad hoc grouping of the King and his senior military commanders which managed the Kingdom of England's wars and campaigns. The management of the War Office was directed initially by the Secretary at War, whose role had originated during the reign of King Charles II as the secretary to the Commander-in-Chief of the Army. In the latter part of the 17th century, the office of Commander-in-Chief was vacant for several periods, which left the Secretary at War answering directly to the Sovereign; and thereafter, even when the office of Commander-in-Chief was restored on a more permanent basis, the Secretary at War retained his independence. The department of the Secretary at War was referred to as the 'Warr Office' (sic) from as early as 1694; its foundation has traditionally been ascribed to William Blathwayt, who had accompanied King William III during the Nine Years' War and who, from his appointment as Secretary in 1684, had greatly expanded the remit of his office to cover general day-to-day administration of the Army. After Blathwayt's retirement in 1704, Secretary at War became a political office. In political terms, it was a fairly minor government job (despite retaining a continued right of access to the monarch) which dealt with the minutiae of administration, rather than grand strategy. Benzodiazepines are usually administered orally; however, very occasionally lorazepam or diazepam may be given intravenously for the treatment of panic attacks. === Causes === The most common cause of periwound issues is excessive moisture present in the area surrounding the wound. Exudate from heavily draining wounds causes irritation of the periwound that may lead to maceration, excoriation, and otherwise compromise skin integrity. This type of damage is more common in chronic wounds due to exudate composition which differs from fluids produced in acute wounds or burns. Chronic wound exudate contains proteolytic enzymes and other components that degrade skin integrity and predispose it to inflammation. Moisture-associated skin damage can also be caused by bodily fluids or other contaminants that enter the periwound areas, for example, in patients with urinary or fecal incontinence, or colostomy patients. Other causes include dryness of the skin due to ageing and skin or systemic disorders, allergic reactions to wound care products, damage that may result from poor application and removal technique of adhesive products used in wound treatment, as well as exposure to infection or extrinsic contaminants at the time of wound dressing changes.

=== Creatinine === In clinical practice, creatinine clearance or estimates of creatinine clearance based on the serum creatinine level are used to measure GFR. Creatinine is produced naturally by the body (creatinine is a breakdown product of creatine phosphate, which is found in muscle). It is freely filtered by the glomerulus, but also actively secreted by the peritubular capillaries in very small amounts such that creatinine clearance overestimates actual GFR by 10–20%. This margin of error is acceptable, considering the ease with which creatinine clearance is measured. Unlike precise GFR measurements involving constant infusions of inulin, creatinine is already at a steady-state concentration in the blood, and so measuring creatinine clearance is much less cumbersome. However, creatinine estimates of GFR have their limitations. All of the estimating equations depend on a prediction of the 24-hour creatinine excretion rate, which is a function of muscle mass which is quite variable. One of the equations, the Cockcroft and Gault equation (see below) does not correct for race. With a higher muscle mass, serum creatinine will be higher for any given rate of clearance.
